Age Concern Coventry, NHS Coventry and Coventry City Council have joined together to ask Coventry University to carry out a major survey of the views of local people, aged 55 and over, and who live in Coventry or are registered with a Coventry GP.

The survey asks questions about a range of things, including your health and well-being, the choices you make about how you live and work, and how you spend your leisure time. Your views and the overall results will help us to provide good services that the older citizens of Coventry would most like to have in the future.

The questionnaire has been carefully designed to be completed quickly and easily and is striclty confidential.
